Iris Photography Consent & Release Form

 

The Greenwich Studio will be taking high-resolution photographs of my iris using specialised photographic equipment and lighting.

 

I understand and acknowledge that:

• The session involves looking towards a bright light source for short periods.

• This may cause temporary discomfort, watering, light sensitivity, or eye fatigue.

• If I have any existing eye conditions, recent eye surgery, light sensitivity, migraines, epilepsy, or other medical concerns, I should inform the photographer before the session.

• Participation is voluntary, and I may stop the session at any time if I experience discomfort.

I confirm that I am participating at my own discretion and that I have disclosed any relevant medical conditions that may affect my ability to safely take part.

 

The Greenwich Studio will take reasonable care when conducting the session but cannot be held responsible for temporary discomfort or reactions resulting from participation, except where liability cannot legally be excluded.
